Rewards for participating in this wellness program are available to MCJHIF health plan members (Aetna, Cigna, and Horizon Blue Cross Blue Shield) who meet the program requirements. As part of the program, you’ll get assistance in managing your health from diabetes educators, helpful diabetes care reminders, healthy living tips and recipes, and educational resources. You can also get all your covered diabetes medications and supplies at NO COST, that includes both brand and generic medications and testing supplies for the first 90 days while you work on meeting the program activities with your doctor/s. The Good Health Gateway also offers additional rewards such as Amazon gift cards throughout the year for enrolling in the program and meeting diabetes care requirements.
