Recently enacted Federal and State laws have brought a number of changes to the financial services industry. One result of the legislation is that financial institutions and insurance companies, including the Middlesex County Joint Health Insurance Fund ("Fund"), are required to disclose how the institution treats information it maintains about participants.

provided to your employer every year and maintained on our web site. If you have
any questions about the policy throughout the year, please feel free to ask your

Middlesex County Joint Health Insurance Fund's Privacy Policy
Middlesex County Joint Health Insurance Fund ("Fund") believes in protecting the confidentiality and security of all Nonpublic Personal Information that it collects about its participants. Nonpublic Personal Information generally means information that is provided by our participants or obtained by the Fund in the course of managing the Fund and processing claims. It does not include information available to the general public.
Collection of Nonpublic Information by the Fund
The Fund collects information for business purposes with respect to providing health benefits to Fund participants. These business purposes include, but are not limited to, purchasing reinsurance, evaluating, managing, and paying claims, and processing of claims.
The Fund collects this information from the following sources (examples under each source are not necessarily a complete list):

Application Information: Information the Fund receives concerning participant on applications, or other forms. It may include participant's name, address, and telephone number, social securit number, date of birth, length of employment and gender.

Claim Information: In the event that there is a claim, the Fund may collect other Nonpublic Personal Information. This includes reports concerning participant's condition and treatment including medical reports, personal financial information and information concerning dependents.
Disclosure of Nonpublic Information by the Fund:
The Fund will disclose only that Nonpublic Personal Information it believes is necessary to conduct the business of the Fund or where disclosure is required by law. For example, Nonpublic Personal Information may be disclosed to others to enable them to provide business services for the Fund, such as evaluating requests for insurance or benefits, performing general administrative activities and assisting in processing a transaction. Information may also be disclosed for audit or research purposes or to law enforcement and regulation agencies, for example, to help prevent fraud. Information may be disclosed to others that are outside of the Fund, such as companies that process data, companies that provide general administrative services, and other insurers. The Fund may also make other disclosure of information as permitted by law.
Protection of Nonpublic Personal Information by the Fund.
The Fund treats Nonpublic Persoanl Information in a confidential manner. Fund Professionals are required to protect the confidentiality of Nonpublic Personal Information. Fund Professionals may access Nonpublic Personal Information only when there is an appropriate reaosn to do so, such as to administer benefits or process claims. The Fund also maintains physical, electronic and procedural safeguards to protect Nonpublic Personal Information, these safeguards comply with all applicable laws. The Fund does not transfer or otherwise exchange or provide lists of its Participants or Information concerning Participants to any company unless necessary to process claims or complete Fund Business.
